New Patient Forms
Completing your forms before you arrive turns check-in into a signature instead of twenty minutes with a clipboard.
Our forms are hosted on a secure online portal. You can complete them on a phone, tablet, or computer, and they arrive with us before your appointment so we can review your medical history in advance rather than reading it while you sit in the chair.
Patient Registration
Your contact details, medical history, responsible party, and dental and medical insurance information. This is the form that takes the longest at the desk, which is exactly why it is worth doing at home.
HIPAA Acknowledgment
A short acknowledgment of our privacy practices covering how your health information is used and protected. One page, and it only has to be completed once.
Our patient forms are securely hosted by our third-party forms provider, so the portal looks a little different from this website.
If the portal will not load for you, do not worry about it — call the office and we will either email the forms to you or have them ready at the desk when you arrive.

We are a fee-for-service dental office that accepts all insurances providing out-of-network coverage, and our insurance coordinator files your claims for you. For treatment you are paying out of pocket, we work with CareCredit and Cherry so larger plans can be spread into monthly payments.

A new patient appointment here is a diagnostic visit and a conversation — not a rushed cleaning and a sales pitch.

Forms review and a real conversation
We go through the history you submitted rather than handing you a clipboard — what has hurt, what you have had done before, what medications you take, and what has made past dental visits difficult.

Comprehensive exam and imaging
A full exam and digital X-rays, including gum health, bite, existing restorations, and an oral cancer screening. Complex implant and full-arch cases may also involve a 3D scan.

Your goals, not just our findings
What you want out of your teeth matters as much as what we see on the images — comfort, function, appearance, or simply getting out of a cycle of emergency repairs.

A written, itemized plan
You leave with a plan in writing, each procedure listed separately with its cost, plus an estimate of your out-of-network benefits. See financial information for how that gets paid for.
Plan on about an hour for a new patient visit. If you are in pain, tell us when you call — urgent problems are handled differently and are covered on our emergency care page.

Languages spoken in the office
Care is provided in English, Spanish, and Persian. Dr. Kambez Shukoor also speaks Hindi. If you would rather discuss treatment in one of those languages, say so when you book and we will make sure the right person is with you.
Parking and getting here
On-site parking is available at the Hastings Condominiums building at 4444 Connecticut Ave NW #106, with metered street parking on Connecticut Avenue and the adjacent residential streets as a backup. Van Ness–UDC and Cleveland Park Metro stations and the Connecticut Avenue bus lines are all close by.
Insurance and how claims are handled
We are a fee-for-service dental office. We accept all insurances that provide out-of-network coverage. Our insurance coordinator, Meryl, will answer all insurance-related questions and will work with your insurance provider to ensure we maximize your coverage for your dental needs. She will also ensure all claims are submitted by our office on your behalf as soon as services are completed.
